 24-Hour Coverage.  In the event of an emergency, there is a doctor available 24-hours a day.  After office hours, your call will be routed to our answering service and the physician on call will be contacted.  If you have a life threatening emergency, call 911. For non-emergent concerns, please call our office during regular business hours.  The doctors and nurses will return your call as soon as possible.  Unless you have an urgent issue, the majority of phone calls are returned at the end of the morning and at the end of the afternoon.  Please leave numbers where you can be reached during these times.

If you need a prescription refill, call our refill line at 303-996-0903.  Please plan ahead and call a few days before you are completely out of your medication.  Refills will not be issued over the weekend unless it is urgent.  Prescription refills may be denied if you have not kept scheduled appointments or we were not the prescribing doctors.  If you have an appointment scheduled we will be glad to call in enough medication to cover you until that time.

 From time to time, you may have questions on your bill.  If you would like to speak to someone in our billing department, please contact us.

If you have questions about a bill you have received, please call 303-239-0309.

• Description of Service

If you have general questions about our billing policies or self-pay contracts, please call our office to speak to Cindi Timmons. Her direct line is 303-996-5614.  Cindi spends an enormous amount of time on the phone with insurance companies on behalf of our patients.  Therefore, please do not be frustrated if you get her voicemail, she will call you back as quickly as possible.

Midtown is happy to provide you with copies of your medical record at any time for you or another medical provider.  In order to obtain your records, we will need written authorization.  Please call us to obtain a “Release of Records” form or download the form here.

If medical records are requested directly from another doctor’s office, they are sent directly to that office at no charge.  If medical records are requested by an insurance company, then the requesting company will pay the medical record copying fee.  If you request a personal copy of your medical records, then a small copying fee will be assessed based on the number of pages of your record.
